Bereavement and Grief Support Groups | Visiting Nurse Home and Hospice

For those who have experienced the recent death of a loved one, Visiting Nurse Home & Hospice provides several Bereavement Support Groups throughout the year. These groups address a variety of topics designed to help, heal and support those who have lost a loved one. These groups are open to the public and free of charge.

Visiting Nurse Home and Hospice

Comprehensive home health care for the sick and disabled. Includes professional nursing, social work, physical, occupational and speech therapy, nutrition, diet therapy and home health aides. Maternal/child health services. Clinics for adult health screening. Comprehensive Hospice team-all services, Diabetes outpatient education and Geriatric care management. First Connections provides free, voluntary, confidential home visits to pregnant women and families with young children, birth to age three. Conduct home visits: Trained nurses, social workers, and community health workers meet with families in their homes to talk about their needs, answer questions they may have, and conduct comprehensive assessments.Link families with medical homes and social services: Home visits are based on the needs of the family. Visit topics may include health education and connections with appropriate healthcare services, human services, and community resources. If a child does not have a medical home, we can help the family find one. Visiting Nurse Home & Hospice can also conduct child wellness screenings and communicate the results with the child’s healthcare provider.
